All participants both face to face and online, regardless of sessions attending, must fill out health questionnaire 1 and emergency contact form 6. If you are a 1-2-1 client then we also require you to fill out additional health questionnaire 2 and we may also ask you fill out lifestyle questionnaire 3. If you are attending a back care course either online or face to face then please fill out back care progress questionnaire 6.
